What P0131 means

The upstream oxygen sensor on Bank 1 — the one whose readings actively steer fuelling — is reporting persistently low voltage: reading lean all the time, or electrically failing to a low state. Since fuel delivery follows this sensor, the mixture control is compromised.

What does 'Bank 1, Sensor 1' actually mean?

Bank 1 is the cylinder bank containing cylinder 1; Sensor 1 is the sensor before the catalytic converter (Sensor 2 is after). So this is the upstream, fuel-control sensor on the primary bank — the most consequential O2 sensor on the car, which is why its codes matter more than downstream ones.

Should I replace all my O2 sensors at once?

On a high-mileage car where one upstream sensor has died of age, replacing its same-age partner upstream sensor (on V-engines) is defensible — they share a lifetime. Downstream sensors only monitor the catalysts and can wait for their own codes. 'All four while we're in there' is usually over-selling.
